FL-140 is the cover page identifying financial information exchanged with a spouse or domestic partner as preliminary or final disclosures.

Using this form

Do not file the Declaration of Disclosure or its financial attachments with the court. Read the form’s instructions about the documents to exchange and applicable exceptions. FL-141 is the separate declaration used to report service or an applicable waiver to the court.
